My 9 year old has had 2nd toe nail thickness for a few years

My 9 year old has had 2nd toe nail thickness for a few years on both feet. It looks like it'll grow out but never fully looks like a normal nail. He is narrow footed and ties his shoes so tight I wonder if it squishes his toes. I get him wide toe box shoes and buy new socks often. He also starts to run on his toes despite me trying to teach him different. Long story short, his one 2nd toe nail is looking more off to me where it may not be a tight shoe issue. I know looking at
