Added another photo, its just getting worse.

Added another photo, its just getting worse. Anyone know what this is? I have been seeing a podiatrist for a year now. Had a torn plantar plate about 5 or 6 years ago, had that repaired (2nd toe). Started having pain in the ball of my foot (big toe). Went in and they said there was a fractured sesamoid. Was in a boot for a couple months. Ive been in and out trying to figure out what is causing the swelling and pain that is steadily getting worse. Ive done custom orthotics.
