When did y'all decide it was time to remove a tailors bunion surgically?

When did y'all decide it was time to remove a tailors bunion surgically? I just recently got one maybe 4 months ago, and it's progressively gotten worse. I went to a podiatrist where they did x rays and they recommended I get a sole pad, some of this arthritic cream that takes months to work, and a little toe splint thing but I can only wear it when I also have a lidocaine patch on it because it still hurts. Is it worth going through all these steps to help relieve pain or should I just go ahead and consider surgery?
