I have really bad metatarsalgia due to the long bones of my second and third toes having worn away.
I have really bad metatarsalgia due to the long bones of my second and third toes having worn away. Said bones are longer than my big toe, just how my feet have grown, and they have been taking all the impact for years, I’m 52. Drs have said it’s just cushioning and pain management from now on, there is nothing that can be done. Now the question, im a firefighter and need to source good firefighting boots and work shoes that have great arch support, cushioning and a wide toe box. All suggestions very welcome or it may be the end of my career!

how many times does it take to get custom orthotics to fit good
depends on your feet and the person adjusting. Last set i got was 3 visits. First set was never that way I wanted, but I moved 12 hours away , so couldn't keep going back. Return fittings should be covered in your purchase.
Go see your local pedorthist... they can handle all of that.
I wear Apex not just wide but higher toe box so orthotics w cushion fit. Good luck.
Keen boots have a wide toebox and have enough structural support for my feet. I replace their insoles with something like Powerstep Pinnacle with Met Support and then I also add a Pedag T-form metatarsal Support underneath the insole. I was introduced to all these products by a pedorthist that owned a shoe store. I went in saying I needed custom orthotics and heard he could make them and he said try this stuff. Been using various combinations for at least 15 years now and it’s saved my life. I started with the Keen shoes and have tried the insoles plus extra supports in other shoes. It’s also worked in some Kurus and Altras. The key seems to be that the shoe needs to have enough structure in the sole. It’s a lot less expensive than custom orthotics for every pair of shoes you own and you can self adjust the location of the extra support as you need to. Powerstep Pedag
