Thank you, that is all very helpful. I am actually having Orthotic shoes made so that is a positive. I have tried inner soles from the hospital but did not really help. I will give the metatarsal pads a try also the knee exercises as I have severe arthritis in my knees. Oh the joy's of old age. Once again thank you for your excellent advice.

Ball of foot pain from fat pad loss: cushioning solutions
I have been told by podiatry surgeon that the fat under the right ball of my foot has gone. I have had awful pain when walking. To help with the pain my hammer toe was removed to allow my toes to spread out more!!. I am however still getting pain when I walk. I was advised to get trainers that were well cushioned but so far nothing had helped. The surgeon says that nothing can be done only cushioning but nothing seems to have helped. Does anyone suffer with this and have you any suggestions please.

I don’t know if I had same as you but I saw a lower foot doctor, not a podiatrist and a pedorthist. I got custom orthotics and Hoka runners. They referred me to a physio who did laser therapy and was 100% healed.
The only part of my body that is not too fat is the fat pad on both feet! My new shoes, Gdfy, are very helpful.
Did you try the Hoka bondi? It’s gotta be the most cushioned shoe on the market. Also make sure your arch is supported so your heel doesn’t strike as hard.
In addition to orthotic and shoegear modifications, Find a podiatrist that might be familiar with leneva. It is a fat pad substitute that can be injected into areas of fat pad atrophy. I’ve had some success with it.
Try Fit Flop shoes. I can wear most Fit flop shoes with micro wobble board soles and have no pain in ball of foot while wearing them.
Water filled insoles are heavenly to me underfoot but I don't have what you have in particular, aside from underfoot pain.
